Here is a list of the partner organizations and their responsibilities and contributions to the Safety Net Initiative. Then number next to each organization is their percentage allocation of the current funding award.

The percentage allocation of the funding award does not reflect the millions of dollars of in-kind support and infrastructure provided by the Los Angeles County Department of Health Services that serve as the foundation of this initiative. Also not reflected are the hours of in-kind support provided by all partner organizations.

• Community Clinic Association

  • Coordinate/troubleshoot with clinics on i2i and ESS training and development
  • Provide development consultation for additional i2i funding
  • Support project management
  • Distribute financial support to clinic members/participants

• Los Angeles County Department of Health Services

  • Develop interfaces between i2i and ESS
  • Deploy ESS in the clinics
  • Test and assure data integrity
  • Modify ESS for enhanced data

  • Develop HL7 specification for interface development
  • Interface with participating clinics practice management system

• Evaluation Partner

  • Develop evaluation design for project
  • Collect data
  • Analyze data and write report

• Health-e-LA

  • Provide project management oversight Phase One
  • Report milestones and troubleshoot as needed
  • Facilitate planning for Phase Two development
  • Develop proposals for Phase Two project

  • Facilitates contract development and execution
